We received a draft participation agreement — can we sign it?
Please do not sign the draft version – it still has a draft watermark. It was sent in draft form so your legal team can review it. Once you are ready to execute, notify the Converge Health team and provide the name of your authorized signatory. They will send a clean version for electronic signature.
What should we select in the Statement of Work attachment of the agreement?
Select the services that reflect your organization's intended use of the HIE. The Statement of Work covers a range of options, including clinical data exchange, event notifications, care coordination tools, public health reporting, and more. You are not limited to a single category, and you are not required to activate all selected services immediately. Services are activated collaboratively and in a prioritized order based on your organization's readiness.
Fee offsets are available for participating organizations that contribute data to the HIE, which is a requirement for offset eligibility. For the first year, Converge Health Iowa intends to offset fees for all services. Public health reporting fees are separately offset by the State of Iowa for the first year, with a planned 90% state offset continuing after that period.

Our organization shares an EHR with another organization. Do we need two separate agreements?
Yes, two separate participation agreements are likely required since they are two separate legal entities. However, if the data is coming through one electronic system, a single feed can serve both organizations — as long as there is a way to distinguish the two via a location code or facility code in the message. There are some exceptions depending on the structure of your organization. Reach out to discuss your specific setup.
Can we limit our participation to only ELR and syndromic surveillance or Query Portal Access?
Yes, the participation agreement can be scoped to just ELR and syndromic surveillance and/or Query Portal Access. However, data contribution is required from all participating organizations in order to be eligible for the fee offset. Converge Health needs meaningful data participation from everyone to ensure we can support whole-person care across the state.
