How much does participating in the HIE cost?
Converge Health Iowa uses a transparent, service-based fee structure. Fees are tied directly to the services your organization receives and activates, so you only pay for what you use. No fees are assessed for any service until it has been activated, and no fees will be charged to any participant during the first 12 months of their agreement.
After the initial period, fees are based on your organization type and the services you have activated. An illustrative rate card is included in Attachment 4 of your Participation Agreement. A comprehensive official rate card is forthcoming, developed in collaboration with participants, provider associations, and the State. Organizations that have already signed will not face any surprises when it is published.
Fee offsets are available for participating organizations that contribute data to the HIE. Data contribution is a State requirement and a condition of offset eligibility.

Is the fee structure different for critical access hospitals?
Yes. Critical access hospitals are classified in Tier 1, the lowest fee tier, alongside other small rural hospitals with 25 or fewer licensed beds. This reflects their size, patient volume, and the resource constraints that are common in rural healthcare settings.
As with all participants, no fees are charged during the first 12 months of the agreement, fees apply only to services that have been activated, and fee offsets are available for organizations that contribute data to the HIE.
What is the pricing model for portal access?
Portal access is priced as an annual subscription and varies by organization type. Current rates are:
Hospitals, health systems, and FQHCs: $1,500 per year for up to 10 users; additional users at $100 per year each
Independent practices, rural practices, skilled nursing facilities, and behavioral health providers: $1,000 per year for up to 10 users; additional users at $75 per year each
Health Plans/Payers: Portal access is included with event notification services; up to 10 accounts included, with additional users at $150 per year each
Local public health departments: $1,000 per year per 10 users
State public health laboratory and state agencies: No fee while the State contract is in effect
As with all services, no fees are charged during the first 12 months of the agreement, and fees apply only once portal access has been activated. These rates are illustrative and an official rate card is forthcoming.
What is the pricing model for Event Notifications (ENS)?
Event notification services are priced on a per-member-per-month (PMPM) basis. Current rates are:
Hospitals, health systems, FQHCs, behavioral health providers, independent and rural practices, and skilled nursing facilities: $0.08 PMPM; $0.05 PMPM for organizations with panels exceeding 5,000 attributed patients
Health Plans/Payers: $0.09–$0.15 PMPM on attributed members; ADT feeds are the baseline, with CCDs and ORUs available as optional additions
Local public health departments and state agencies: No fee
With the exception of Health Plans/Payers, no fees are charged during the first 12 months of the agreement, and fees apply only once event notifications have been activated.
